For years, the only truly effective way to tighten skin was through surgery like a facelift or eyelid lift. Thermage technology isn’t new, but the “FLX” aspect of it is. Let’s take a look at how it works and why Dermatology Times touts its benefit as a “one and done” procedure.

Thermage FLX is the easier, faster monopolar radiofrequency (RF) device that is FDA-cleared to be used on both the face and body. This includes the eyelids, a delicate area that not all technologies can treat. In most cases, clients need just one treatment to achieve optimal results, with maintenance sessions recommended every 1 – 2 years. The Cosmetic Bootcamp (CBC) was one of the first to report (in 2018) about how well Thermage FLX works in just one treatment. According to the report, it’s the “treatment that everybody is doing, but many don’t talk about it.” Ultimately, Thermage FLX just isn’t covered in the media as often as other procedures, such as CoolSculpting, so it’s not a household name—yet.

Benefits of Thermage FLX

When it comes to Thermage FLX and the media, another reason it may not be as well-covered is because of the advancements. Years ago, the original Thermage (less the “FLX”) was considered intolerable by some clients. Plus, it was “overly promoted” according to the report. However, it was also the first capacitively coupled RF, monopolar device that was approved by the FDA for a non-invasive treatment of facial and body wrinkles (as well as a temporary improvement in the appearance of cellulite). It is also the only RF tech that is a single-treatment approach featured in peer-reviewed studies with long-term results. There are a few other technologies that also boast one treatment, but they are not RF-based—and radiofrequency is a proven method of collagen induction therapy.

It is also the first to hold a patent for healing dermal tissue via cross-polarization. According to the report, “It also has a reverse thermal treatment gradient, so the RF energy is delivered uniformly over the tip.” This lets your provider use a stamping technique instead of a solid, linear motion, which improves comfort while ensuring uniform application. The reverse thermal gradient is how heat is delivered to the subcutaneous level while making sure the skin surface stays cool. Bipolar RF was the original approach, but monopolarity also allows the energy to go deeper, improving results.

A History of Heating Things Up

The multi-pass NXT protocol is what paved the way for making Thermage treatment more tolerable in 2007. However, the real change came when vibration was added in 2009. “This is the gated theory of pain,” explains the author. “Just as the dentist grabs your mouth before injecting the needle, the first thing patients feel is vibration, then RF.” The Faster Algorithm Experience, or FLX, features automatic calibration in the latest generation. Today, Thermage FLX treatments are 33 percent shorter than the original Thermage. On average, a face and neck treatment takes 60 minutes. One doctor says, “The first question I ask is, are you considering anything surgical?” Most people seeking out Thermage FLX already know they either don’t want surgery (at all) or aren’t ready for it. Typically, Thermage FLX coupled with other procedures like Botox and/or dermal fillers can be a fantastic way to dramatically turn back the clock without any surgical interventions. The term “tech neck” is used to describe excessive lines, wrinkles, laxity, and banding of the neck due to routinely looking down at our tech devices. Research has found that tech attachment is starting at younger and younger ages, and this is starting to show in our skin. In fact, one report shows the average American spends 5.9 hours each day with digital media engagement. Step 2: Botox

In 2021, researchers released the paper “A survey on monopolar radiofrequency treatment: the latest update” in Dermatologic Therapy. Their goal was to measure the perceived effectiveness and side effects of some of the most popular RF treatments used by Korean dermatologists, including Thermage FLX. In total, there were 52 surveys analyzed, all of which included Thermage FLX, and by far (57.4%) Thermage FLX was considered the most effective treatment. It scored as the shortest treatment time but was also considered the most uncomfortable. Fortunately, at Palo Alto Laser and Skin Care, we take measures to ensure the most comfortable treatment experiences possible.

Taking an over-the-counter painkiller, like Advil, is permissible before Thermage FLX. Additionally, a topical numbing cream is applied. With these measures, most clients describe the feeling as being like a burning or prickling sensation. Additionally, the passes Thermage FLX requires are fast, so any sensations are quick. The best candidates in these surveys were considered to be those in their 40s. The degree of skin thickness was determined to be the most important factor in responsiveness. Loose skin can happen anywhere on the face or body and can be caused by a myriad of reasons, including decrease of collagen production, genetics, gravity, and trauma (such as routinely rubbing the eyes). The right treatment—or treatments—for loose skin depends on severity, location, and cause. One of the best treatments for loose skin is Thermage FLX, which uses radiofrequency (RF) to heat the deep layers of tissue below the skin’s surface as a form of collagen induction therapy while leaving the top layer intact. Results are astonishing and typically just one treatment per site is needed for results that last up to two years. One of the best perks about Thermage FLX is that it can be used on larger areas such as the stomach. The outcome is so impressive that many clients find it an excellent alternative to more aggressive, invasive procedures such as cosmetic surgery.

While Thermage FLX is a popular solution for treating loose skin in many areas, it’s not the only option—especially if you’re concerned about small and precise areas like around the eyes.

Fillers and Botox for Skin “Tightening”

Sometimes “loose skin” is actually areas of hollowness, particularly in the face. As we get older, we lose not only collagen but also volume. Simply “tightening” the skin through a method like Thermage FLX isn’t going to give you the results you want because what’s needed is a restoration of volume. That’s where dermal fillers and boosters come in.

Dermal fillers are typically made of synthetic hyaluronic acid (HA) and literally “fill” the areas where volume is missing. Results are instantaneous, though some redness, swelling, and bruising may be visible for a few days. HA fillers can last anywhere from 6 – 18 months, depending on the treatment site, units, and type of fillers. “Boosters” are a filler that is not made with HA and depends on collagen induction. It’s a subtler, slower approach that may be desired by some clients in some areas (such as the tops of the hands).

Botox and other neuromodulators can also help in some areas where “skin tightening” is required. These injectables stop wrinkles and lines in their tracks by temporarily paralyzing the muscles that cause them. Skin might not be necessarily “loose,” but rather muscles may have fallen into the habit of creating wrinkles—and in turn the appearance of loose skin.
