During the summer months, many people want to avoid treatments that can make skin hyper-sensitive to the sun such as chemical peels. That’s a smart move, and at Palo Alto Laser and Skin Care we offer a variety of treatments that are summer-friendly. The triple threat of injectables can be used year-round, and if you’ve only tried one of them before it might be time to see what all three can do. First up is Botox, the gold standard in stopping wrinkles in their tracks. However, there’s a lot more to this neuromodulation.

Botox has been around for decades and works by temporarily freezing or paralyzing targeted muscles that cause lines, wrinkles, and folds. Increasingly, it has been being used by younger clients for preventative measures. If you can stop lines and wrinkles from prematurely forming—prevention bests a cure every time. Botox can also be used to produce the illusion of a fuller upper lip while masking a gummy smile. This is done via a lip flip, where small amounts of Botox are injected into the upper lip area. In an era when everyone is looking to save time and money, the at-home cosmetic treatment is at an all-time high. However, there are some procedures and treatments you only want to let a professional handle—like chemical peels (of any strength). Light and medium-depth peels are a great way to quickly and evenly address a variety of skin issues from fine lines and wrinkles to dullness and brown spots. It used to be a given that getting a peel would take place in-office, but thanks to COVID and social media there is a boom of at-home treatments. In the best-case scenario, they are pointless. But worst-case scenarios can result in serious consequences.

All chemical peels use at least one chemical to literally peel away the top layer or layers of skin. Very light peels might not even cause actual peeling, but flakiness is common. Medium depth peels will (or should) definitely peel. The types of chemicals used and their strength all dictate what the peel is called. The depth is related to how many skin layers are removed. Deep peels are also an option, but are typically only for very severe skin conditions and are performed under anesthesia in an OR setting.

What Chemical Peels Treat

One of the reasons everyone loves a peel is because they are so versatile. They can handle wrinkles, scarring, laxity, and much more. Peels are evenly applied to the entire face, so even if you’re mostly concerned about brown spots from UV damage, you’ll be getting the benefits of a dewier, glowing complexion everywhere. Sometimes peels can be tried on the neck, chest, and other areas but this typically isn’t very effective. You need a certain amount of oil glands in order for peels to work, and that is only possible on the face (still, it works for some people, so you can always add on extra peels to areas besides the face and see if you’re one of the lucky few!).

Many peels require a neutralizer to be applied at a precise moment after application. Others, like TCA peels, do not. This can inspire some people to take a DIY approach—after all, if a peel self-neutralizes, what could go wrong? Peels might seem fast and easy, but it actually takes months or even years of training and experience for an aesthetician to become a skilled provider. There are risks with any procedure, but trusting a knowledgeable provider will keep you safe and achieve the best possible results.

Prepping for a Peel

Most of the at-home “peels” you see on the market are not medical-grade. This makes them much safer to self-apply, but it also means you’re not going to get the kind of results you want. After all, technically there are many chemicals that can be used for a “peel,” and that can include things like citric acid. If you really want to visibly improve your skin, you need a medical-grade peel that is only legally available in office settings.

The first step in a pre-treatment assessment is to understand your needs, goals, and any current medications. Some medications and medical issues can mean a client isn’t a good fit for a peel, but might be a great fit for alternative options like lasers. Prior to getting a peel, it’s important to stop retinol use for a set period of time, avoid the sun, and stop waxing, getting facials, exfoliating, bleaching, or undergoing laser skin rejuvenation. Those who are prone to keloids, who do not heal well, or have immunosuppression are often not good candidates for peels.

During your appointment, your aesthetician will clean the skin and apply petroleum jelly to areas around the eyes, nose and mouth that are sensitive and should not be peeled. Next, they apply the chemicals to the least delicate areas such as the forehead and cheeks, feathering the peel outward for a natural outcome. Dry swabs are on hand to wipe away tears (which are common due to the peel itself, not pain). Syringes are always on hand, full of saline in the rare instance that the peel gets into the eyes (which almost never happens in clinical settings). Finally, your provider will go over aftercare instructions—which also includes avoiding the sun and retinol medications.

Chemical peels might be the best solution for your acne scarring, and this procedure works well in tandem with other treatments such as microneedling or laser skin rejuvenation. Acne is a medical condition which can be serious, and frustratingly enough it doesn’t always end when the breakouts are controlled. Acne scarring is common and can lead to self-esteem issues for many patients. Oftentimes, these scars are permanent, particularly for those with cystic acne.

Acne scars are a result of inflammation and can occur even if you don’t pick your pimples, and even when you follow all acne treatment best practices. There are two kinds of acne scars, atrophic and hypertrophic. The atrophic kind create a depression or dent in the skin because the fat, collagen, or other tissue at the site is damaged. Contrarily, hypertrophic scars are raised and result from an influx of collagen, or the body’s attempt to over-treat the trauma. Chemical peels can help with both kinds of scarring, but are best reserved for atrophic scars.

How Peels Work

Chemical peels are a type of exfoliation comprised of various chemicals. It’s a type of controlled injury and your aesthetician will select the best peel for your skin and kind of scarring—some peels are not suitable for those with more melanin, but we have peels for every skin tone. Peels literally peel away the top layer or layers of skin, effectively removing acne scarring. At the same time, peels speed up skin cell turnover to stimulate collagen production. This means the results of a peel keep getting better in the days and weeks following the treatment. There are three grades of peels—light/superficial, medium, and deep—though the vast majority of peels used today are light or medium. The first two kinds require little to no downtime, while deep peels are often administered by surgeons under anesthesia because they are painful and require a long recovery period.

Peels are considered the first line of treatment for atrophic scars, but are only sometimes used for hypertrophic scarring. This is because peels on hypertrophic scars can sometimes make the scarring worse. For hypertrophic scars, a better approach might be laser skin rejuvenation. However, if you have atrophic scars, a medium-depth peel is usually the best bet. Scars are often deeper than light lines and brown spots, so you’ll want a more aggressive peel that can actually remove the layers of skin holding the atrophic scars.

Treating Multiple Skin Issues

You’ll probably treat more skin concerns than just acne scarring if you opt for a chemical peel. Peels can also be effective in addressing sun damage, fine lines and wrinkles, hyperpigmentation (including melasma), and even razor bumps. Those with sensitive skin or conditions such as eczema, seborrheic dermatitis, and psoriasis should heed extra caution when thinking about a chemical peel. Peels can trigger or worsen these conditions and healing might not be as desired. You can talk about your dermatological medical history and concerns during your consultation to see if a peel really is right for you and your acne scarring.

There is not a lot of pre-treatment prep, though you should avoid the sun and certain topicals like retinoids for a few days before treatment. After your peel, continue to avoid the sun since your skin is now “new” and vulnerable. A gentle cleanser and moisturizer twice per day will speed up healing. Some peels can cause flaking and it’s critical not to pick at these flakes—as this can result in scarring or hyperpigmentation and even infection. Skin requires a full cycle of recovery, typically 28 – 45 days, to regenerate itself. Until that time, skin is raw even if it looks healed.

Acne aftermaths can last forever when hyperpigmentation is left behind. At Palo Alto Laser and Skin Care, we know that chemical peels are one of the best ways to remove brown spots and light scarring left behind by acne. But which peel is the “best?” That was the question behind research published in The Journal of Clinical and Aesthetic Dermatology. There are many different peels available, but little research has been done that compares the efficacy of each. This study, published in 2022, compared two of the most popular peels: alpha and beta hydroxy (salicylic) acid peels, specifically for post-acne hyperpigmentation.

In total, 40 patients with this condition were split into two groups. Those who received a GA peel got 50 percent glycolic acid (GA) and those in the salicylic acid (SA) group received 30 percent of the acid. This procedure was done every two weeks for a maximum of six weeks. The “primary outcome” was recorded once every two weeks. A blinded evaluator managed the photographic assessment. The mean average age of the participants was 23.29 years old and there were 24 females along with 16 males. Both types of peels show “significant improvement” beginning with the first follow-up.

The Best Peel for Acne

Even though both peels performed well, the GA peel did show more reduction in post-acne hyperpigmentation during every follow-up appointment. The GA group enjoyed over a 75 percent reduction in hyperpigmentation in most patients. No participants in the SA group achieved such success. The researchers say, “Our results suggest that the GA peel was more effective than the SA peel in improving postacne pigmentation.”

Many people will have acne in their lives, and hyperpigmentation caused by this skin condition is common. Acne vulgaris can be brought on by a myriad of factors, including microbiological, immunological, and hormonal. It can happen to anyone at any age, though it is most common in teens (85 percent). The next most common group to be affected are those 25 – 32 years old (8 percent). Chemical peeling is a relatively affordable treatment for acne pigmentation and requires little downtime. However, skin color must be taken into account when choosing the right peel for you.

What to Know About Peels

If you have never had a peel before, the good news is that we have come a long way since the 1990s, when deep peels were the norm. Today, superficial and medium-depth peels are what most people receive. This means little to no downtime for light peels and a relatively quick recovery (1 – 2 weeks) for medium peels. When getting a peel, the process is quick. Skin is cleansed, the chemicals are applied, and then they are neutralized after just a few minutes. You may feel a slight stinging during application and/or neutralization, but it is not necessarily painful. In some cases, such as with very superficial peels, you do not even have to remove/neutralize the peel.

After-care is critical to protecting your skin and getting the best results. Your provider will tell you exactly how to care for skin in the delicate, peeling state. This may require applying certain topicals and every peel will require that you avoid unnecessary sunlight. As skin peels away, it takes the affected skin cells with it, including hyperpigmentation. This reveals younger looking, clear, glowing skin below the surface while also stimulating collagen and elastin production. Most people see optimal results in 2 – 3 weeks, but skin will keep improving from the inside out thanks to collagen production.

Rosacea is an incurable inflammatory skin condition that can affect people of any skin tones. The key to preventing and minimizing flare-ups is to avoid triggers, and for years many people thought that treatments like a chemical peel were a triggering no-go for those with rosacea. The treatment is not inherently contraindicated for this condition , but there are some things you need to know.

Obviously, you want to enjoy the same beauty treatments as everyone else to get clear, glowing skin. The general rule of thumb with rosacea is to avoid over-exfoliating. The chronic state of rosacea means that your skin barrier is compromised, so an easy “fix” some people do is avoid exfoliation entirely. Chemical peels range in severity from mild to aggressive, but are clearly more of an exfoliant than an at-home scrub. However, there is a big difference between a chemical peel and your tube of exfoliating cream: Chemicals peels should be performed by a professional in an office setting.

The Difference a Professional Touch Makes

Of course, there are some rosacea patients who should avoid chemical peels. Everyone is different, so if you have rosacea and are considering any kind of treatment, then a consultation is necessary. For a lot of people, a gentle peel can actually help in managing the condition and can pair well with other treatments. The trick is getting matched with the right peel and to be treated under the right conditions.

It’s estimated that about 10 percent of people have rosacea. It can present in different ways and there are also different types of rosacea. One of the most common types presents as “just” redness while the other is dubbed “acne rosacea.” A less prevalent sort causes a thickening of the skin, and then there is even ocular (eye) rosacea. Within this type of rosacea, you might experience different degrees of severity throughout your life. There is also the fact that you can have more than one type of rosacea at a time. Some of the most common rosacea symptoms include flushing, bumps/pimples, sensitive skin, a burning sensation, visible blood vessels, and sore eyelids. With so many types and symptoms of rosacea, obviously working with a pro is critical.

What a Peeling

There are more types of chemical peels than there are types of rosacea. During a treatment, your provider applies a solution comprised of acids in varying pH levels directly to the treatment site. Depending on the type of peel and the depth it reaches, you can expect all different types of aftercare. There is no particular peel that is best for those with rosacea, but a superficial peel with a low pH is what you will need. This ensures that only the very top layer of skin is removed, which is usually comprised of only dead skin and no more. When the right peel is used, it can help to keep breakouts and redness in check while minimizing inflammation. However, a chemical peel alone isn’t enough to manage your rosacea. It should be part of an overarching plan.

Chemical Peels + You

For those with rosacea, a light peel every 4 – 8 weeks is enough. Most people with rosacea need a series of treatments to establish a foundation of healthy skin. Then, maintenance sessions every 6 – 12 months are recommended. If you’re new to chemical peels, right now is a great time to get started on that initial regimen. Sun can both trigger rosacea and interfere with chemical peel recovery, so cooler months like those in autumn are usually recommended for peels and other resurfacing treatments.

Remember that if you have rosacea, skin is already compromised. If you use acid too much, the skin won’t be able to heal and repair itself. Too many peels, too harsh a treatment, and of course, a DIY approach can enhance inflammation, worsen itching, and not allow the skin to heal. Plus, additional issues could arise, such as hyperpigmentation and acne.

What’s the difference between a facial at home and a peel in-office? Isn’t shaving your face the same as dermaplaning? Chemical peels

A chemical peel is completely different than a facial (though there are also in-office facials available, too). Peels come in light/superficial depth and medium depth, and can be comprised of a variety of ingredients, including chemicals that peel away the top layer of skin. A peel removes the top skin layer(s) in order to get rid of superficial issues such as light lines and pigmentation. However, it’s also performing work from the inside out. When the skin experiences a controlled trauma, like a peel, it starts producing more collagen at the treatment site. That’s why the results of your peel continue to look better and better even after any flaking, peeling, and redness has subsided. No peel should ever be performed at home.
