During the summer months, many people want to avoid treatments that can make skin hyper-sensitive to the sun such as chemical peels. That’s a smart move, and at Palo Alto Laser and Skin Care we offer a variety of treatments that are summer-friendly. The triple threat of injectables can be used year-round, and if you’ve only tried one of them before it might be time to see what all three can do. First up is Botox, the gold standard in stopping wrinkles in their tracks. However, there’s a lot more to this neuromodulation.

Botox has been around for decades and works by temporarily freezing or paralyzing targeted muscles that cause lines, wrinkles, and folds. Increasingly, it has been being used by younger clients for preventative measures. If you can stop lines and wrinkles from prematurely forming—prevention bests a cure every time. Botox can also be used to produce the illusion of a fuller upper lip while masking a gummy smile. This is done via a lip flip, where small amounts of Botox are injected into the upper lip area. Results only last about three months on average since this is a very mobile area. Dermal Filler Fun

Dermal fillers are usually made of synthetic hyaluronic acid (HA), with natural HA already occurring in our bodies. HA draws moisture to the area, making it a very natural-looking filler. The results last anywhere from 6 – 18 months on average depending on the type of filler, the injection site, and the number of syringes used. Today’s trend is less about getting fillers in just one area, such as the lips, and more about taking a holistic approach to dermal fillers.

Dimpled chin lowering your self-esteem? We aren’t talking about a cute cleft or singular, youthful dimple, but rather the dimpling that comes with age and lost facial volume. Botox can do just about anything, including giving you back a smooth, younger-looking chin. It’s one of the most popular injectables at Palo Alto Laser and Skin Care (and around the globe). Recently, Johanna Ferreira talked with PopSugar about her chin Botox experience, what it was like, and why she loves the results.

Also known as “orange-peel chin,” dimpled chins are one of the lesser talked about signs of aging. There are some topicals that claim to help, and although moisturized skin is always going to look plumper and healthier, there’s no cream that will correct chin dimpling. When the chin muscles are overactive, or are more visible due to facial volume loss, the only way to stop dimpling is by stopping muscle movement. Ferreira put off Botox in this area for years, scared that it would affect her smile (spoiler: it doesn’t). She didn’t see the dimpling in the mirror, but it started to show up more and more in selfies and other photos. She also compared it to “dimpled resting B face” whenever her mouth was closed (due to unconscious clenching). When she heard that her sister’s wedding photographer specialized in candid photos, she finally decided to do something about it.

Botox for Every Part of the Face

Ferreira was nervous because although Botox is a time-tested solution for lines and wrinkles, there’s not a lot of online accounts about Botoxing the chin. However, Botox is one of the safest in-office procedures. Her doctor explained to her that “an orange-peel chin is the term used to describe a textural change with grooves, dimples, and a pitting appearance of the chin when you are puckering or sometimes resting … the dimpling is caused because part of this muscle is superficial and inserts into the skin of the chin.” In other words, even quite young people can have orange-peel chin, but we are more inclined towards it as we get older.

Ferreira needed just two injection spots on her chin for the best results. It’s critical that only the mentalis muscle is injected with Botox, rather than adjacent muscles, otherwise smiles can be compromised. This is yet another reason to only see a skilled injector for Botox or fillers, such as those at our clinic that are all personally trained by Zuzana Likar. Results from any Botox injection are not immediate, but appear within about 4 – 7 days post-injection. Results are also temporary, lasting 3 – 6 months on average. However, Ferreira noticed some improvement after just one day. One week later, she found her chin to be totally dimple-free and smooth. “The best part was I looked just like myself,” she says, “only without the dimpled chin.”

Chin Up

As a bonus, Botox in the chin is one of the most affordable sites because relatively little Botox is needed. One year later, Ferreira says she maintains her Botox sessions because it has “greatly improved” her chin dimples. Even when the Botox starts to wear off (at about four months for her), the dimpling comes back slowly and subtly. She’s proud to say, “I’m fortunate to be at a place with my self-esteem and body image where there is literally nothing else on my face that bothers me.” She also appreciates taking a preventative approach to chin dimpling, as Botox is known for weakening muscles that cause unwanted dimpling, lines, or wrinkles. When people are young, dimples and wrinkles are dynamic. But as we age, they become more static. Botox can help delay that process.

Injectables refers to minimally invasive treatments that “inject” clients with a variety of products. One of the best-known injectables—and most requested at Palo Alto Laser and Skin Care—is Botox. However, just because Botox has been around for decades doesn’t mean you should trust just anyone with this treatment. The skill of your injector matters just as much as getting matched with the right treatment. Botox is best known for stopping lines, wrinkles, and creases in their tracks. This is achieved by temporarily “freezing” the muscles that cause these unwanted signs of aging. Botox is the most-used neuromodulator and results typically last for 3 – 6 months depending on the injection site and number of units used. Younger clients are increasingly using Botox as a preventative measure to train muscles not to create such deep lines in the first place. However, there are also a myriad of off-label uses for this injectable.

Turning Back the Clock

Botox can do a lot more than address wrinkles. It is also an excellent tool for giving a slight arch to the eyebrows. You can also use small amounts of Botox for a “lip flip,” which essentially flips the upper lip outward so that more vermillion shows. This can help hide a gummy smile and provides the appearance of fuller lips without actually increasing volume. However, for those who do want more voluminous lips—or more volume anywhere on the face—dermal fillers are most often the best option.

The majority of dermal fillers are made with a synthetic form of hyaluronic acid (HA). The HA our body naturally produces attracts water, which is why HA is associated with youthfulness and plumping. The results from dermal fillers are immediate, though some slight swelling and redness is common. Fillers, like Botox, are also temporary, but you still want to make sure you get results you love. “Duck lips” and “trout pout” might be short-term, but since most fillers are in highly visible places you want natural-looking results. An injector without the training, skills, and experience might give you an outcome that is poor and very fake looking.

The Latest in Injectables

There are a few dermal “fillers” that aren’t fillers at all. Instead, they work via collagen induction. Results are not immediate, and it can take up to six months to see results, although slow and steady improvement in volume will be noticed during this waiting period. The benefits of such fillers are that they tend to last longer (up to two years on average compared to the 6 – 18 months offered by HA fillers). Plus, a handful of clients prefer an “all natural” approach to volume improvement. You can also achieve collagen induction with a myriad of other treatments, such as microneedling. Non-HA fillers are less common and newer than HA fillers.

Another recent development in the world of injectables is Kybella. This procedure relies on a series of synthetic deoxycholic acid injected using a specialized stamping device. It treats unwanted fat in the submental (under-chin) area. On average, about 3 – 6 cycles spaced one month apart are needed for most clients. Conservative injections are given because it’s crucial not to over-correct. Once fat is gone with Kybella, it’s gone for good. Nearby fat cells can expand if weight gain occurs, but the specific cells burned with Kybella will not return. It’s important to know if double chins are caused by fat, sagging skin, or both. This can be discussed during your consultation, which is always complimentary. If sagging skin is also a problem, this can be addressed with a variety of in-office skin tightening treatments.

Daxxify is a new neuromodulator that some claim works within hours and lasts twice as long as Botox—which is why Allure editor Lauren Caruso was quick to try it out for herself. However, there’s a reason you may not have heard of it and why it’s currently not offered at Palo Alto Laser and Skin Care. Simply put, the jury is still out on Daxxify and, given how new it is, there aren’t enough studies or evidence to show how it compares to Botox. Still, a first-hand account is worthwhile and something to consider if you count on neuromodulators to stop and prevent lines and wrinkles.

For Caruso, she starts by stressing that she’s a “big fan of Botox—huge.” She has used it regularly throughout the past six years, only messing up her Botox routine during the peak of COVID. She says, “Like most users, I love how Botox smooths my fine lines and keeps deeper wrinkles from forming.” However, it also takes Botox 1 – 2 weeks to fully “work” and it can wear off in a few months depending on how many units are injected and the injection site. The FDA just approved Daxxify, and Caruso had heard stories about how well it worked. But what’s the difference between the two injectables?

Neuromodulator Differences

All neuromodulators that have FDA approval report a high patient satisfaction rate. Of course, everyone wants the result to last as long as possible. In clinical studies, Daxxify (also known as Daxi) lasts six months on average. That is longer than Botox and competitors like Dysport, Jeaveau, and Xeomin. It allegedly lasts longer because of the number of peptides in the formula, although this change does not shift how it’s delivering the botulinum toxin to the nerve cells. Instead, peptides can help to foster the interaction between the injectable and the nerve cells. Peptides are positively charged, binding to the core neurotoxin that is negatively charged (along with the nerve cells, which are also negatively charged). Some say this means longer-lasting results.

Also, right now Daxxify is only FDA approved for treating the glabellar lines, also known as the “angry 11’s” between the brows. Botox is approved for many more areas. This may or may not make a difference to users since neuromodulators are typically used off-label, meaning that patients regularly have them injected in other areas. Caruso went to a clinic that has been offering Daxxify since December, and has thus far only tried it on staff as well as “well-established patients with a history of success using other neurotoxins.” The team there also pointed out that only patients who wanted “less mobility for a longer period of time” were offered Daxxify as an option, since that isn’t the goal for everyone.

One Person’s Daxi Journey

Caruso reports, “Because I’m not particularly enthusiastic about looking frozen, [the injector] used a light hand on me—26 units, to be exact—distributed between my forehead (technically an off-label use) and my 11 lines.” She points out that, “This is only 65 percent of the FDA-approved, 40-unit dosage used to treat the area between the brows alone.” Compared that to Botox, whose FDA approval for between the brows is just 20 units. However, FDA approval is largely based on the strength of the neurotoxin. In this case, 20 units of Botox is the “same” strength as 40 units of Daxxify between the brows.

Daxi is anecdotally known amongst injectors who have used it to give a more “immobile” and “glossy” look to the injection site. This is crucial in deciding not only which neuromodulator to use, but how many units. Daxi is not being used for more creative results, such as the lip flip, since nobody really knows how the results would look.

Caruso’s Daxxify Results

There is no real-world data yet to see how long Daxxify works or how quickly you see results. For Caruso, she says within 24 hours “the effect was significant.” She estimates that it was fully in effect after three days of being injected. However, that comes with a price—the cost of Daxxify depends on the location, but on average it costs 50 – 100 percent more than Botox. If cost is a factor in your decision, the best-case scenario is that you will spend the same in a year for neuromodulators.

Caruso says she’s obviously still waiting to see how long her results last. Until then, most people are waiting for others to report on their success rates before trying something so new. Botox has long been the gold standard in stopping and preventing unwanted lines and wrinkles. That’s why it’s one of the most requested treatments at Palo Alto Laser and Skin Care. Although “Botox” is commonly used as an umbrella term for various neuromodulators, it also remains the most popular type of injectable. A recent 2022 literature review published in Clinical, Cosmetic and Investigational Dermatology set out to consider the efficacy of Botox for “reducing facial wrinkles” (although Botox is also used off-label for various other concerns, such as a gummy smile or to lift the brow).

Botox works by blocking the body’s release of acetylcholine, which leads to a temporary paralysis in targeted muscles. Paralysis usually begins to occur 24 hours after the injection with full paralysis at two weeks. The results of Botox typically last 3 – 6 months. Researchers found that the “optimal dose” of Botox is 20 units per site. “However,” writes the team, “in certain circumstances, the effect of Botox will gradually resolve, resulting in reduced muscle paralysis over time.” This means some tolerance is common, but also exacerbated by the body’s natural increase of wrinkles and lines with age.

Is Botox Effective?

“Botox is a good and safe medicine to reduce the appearance of facial wrinkles,” the researchers concluded. It has been used since the 1970s in the ophthalmology field and more commonly in the past 20 years for a variety of health and beauty concerns, including in dermatology. Botox for facial wrinkles, and particularly the angry 11’s between the brows, received FDA approval in 2002. Another type of Botox, created in France, was approved by the FDA in 2009. The researchers traced their review back to a 1994 study which explored the effectiveness of Botox A (the first type approved by the FDA), which touted it as a good option for reducing facial wrinkles—numerous studies since then have supported these findings.

Today, of course, Botox is not used to just treat the glabellar lines. It can be injected virtually anywhere in the face or body, and in the face it is often used around the lips for smoker’s lines, for marionette lines, the platysma bands in the neck, horizontal lines on the forehead, for strabismus, and various other problem areas.

The research team also wanted to compare Botox to dermal fillers and non-invasive options like facial muscle exercises. They looked at many of the widely used hyaluronic acid (HA) fillers, which are comprised of a synthetic form of the acid that is naturally found in the body to provide plumpness and volume. Both the “real” and synthetic HA attract water, which leads to an attractive swelling that can still withstand compressive pressure. This makes it ideal as a dermal filler.

Comparing Botox and Fillers

When it comes to contraindications, they are similar between Botox and fillers (such as those with bleeding disorders or hypersensitivity to the ingredients that make up the products). However, although injector skill is critical for both Botox and fillers, the researchers stress that “the use of filler is very dependent on operator expertise [and] there are several side effects associated with injecting techniques such as unrealistic patient expectations, undercorrection, overcorrection and lumping.”

Lumping is something that is more common with fillers than with Botox. It occurs when the placement of filler is incorrect and/or there is lack of injector massage. This can result in an unattractive, bumpy appearance that can take several months to dissipate. Still, the research team notes that for both Botox and fillers, clients should take every measure to choose an injector who is reputable and skilled.

The researchers also looked at facial exercises to reduce facial wrinkles, noting that while it’s completely non-invasive and can be done without professional expertise, it takes constant work. Getting Botox is as common as getting a manicure for many people, but are you using this neuromodulator to its maximum capacity? A 2021 article in Journal of Oral Biology and Craniofacial Research evaluated the longevity and effectiveness of using Botox “in the management of a gummy smile”—in other words, a lip flip. This is a popular procedure at Palo Alto Laser and Skin Care, though, of course, using Botox to prevent and manage lines and wrinkles is much more common. The researchers considered a longitudinal study with a four-year follow-up to gauge whether or not the lip flip was ultimately “worth it,” though a four-year follow-up might be considered foolishly hopeful. Botox for wrinkles lasts six months on average, and a lip flip typically lasts 3 – 4 months. Botox is designed to be temporary and doesn’t last as long in mobile places like the lips.

We know the lip flip works and we know the average timeframe of these results. What is of interest is what the research showed when it comes to assessing if this minimally invasive procedure might be useful as the only treatment to reduce a gummy smile. Subjects included in the study were all willing to get the lip flip, regardless of whether a surgeon had suggested another method. In total, there were 32 subjects divided into two categories. Group one had a gummy smile less than 5mm and were treated with three units of Botox. The other group had a gummy smile larger than 5mm and they received five units of Botox. Each group had two cycles within seven months (a proper timeframe) and kept in touch with researchers for the first 14 months.

Flipping Out Over Botox

All patients tolerated the procedure well according to researchers. There were no allergic reaction or antibody formation associated with the treatment. Both groups demonstrated “excellent” results for three months after the injection. At that point, all reported their gummy smile slowly returning. The authors note that “significant changes” appeared at the five-month mark. This is in keeping with what is already known about the lip flip and while it is not surprising to providers that the lip flip gives fantastic results and requires quarterly treatments, it might come as a pleasant surprise for those looking for an alternative or complement to lip fillers.

In conclusion, the researchers “recommend use of BT [Botox] for the treatment of a gummy smile as the technique is safe, economical and easy to use.” They point out that it is not long-lasting and that it might encourage patients to opt for a surgical fix in the future. However, there are options other than surgery for this common condition. Dermal fillers and “fillers” that often fall into this category but are actually collagen induction therapy can also work. It all depends on the severity of the gumminess, the client, and the look they want to achieve.

Botox Does it All

You might start to notice loose skin, or skin laxity, as early as 30 years old. It is one of the most common complaints we hear at Palo Alto Laser and Skin Care, and why we offer a variety of treatment options. Skin laxity is caused by a decrease in collagen production, decrease in “good fat” in the face (which is what causes that hollowed appearance), as well as gravity and genetics. There are also “fillers” that don’t provide immediate results like HA based ones, but instead work via collagen induction therapy. They take some time for results to show, but offer results that last longer than HA fillers (up to two years).

3. Botox.

Botox to freeze lines and wrinkles that highlight skin laxity. In some cases, you can’t tell skin is lax if wrinkles, lines, and creases are gone!
