How can so many celebrities have glowing, youthful skin with apparently no downtime (as many of them are captured in photographs on a near-daily basis)? The answer may be Fraxel Dual, a relatively gentle laser with little to no downtime. Unlike its predecessor, Fraxel CO2, the new options is not nearly as aggressive which means you don’t need to take time off of work (or off social media) to recover. It is also one of the most-requested laser treatments at Palo Alto Laser and Skin Care, where every provider is trained by expert Zuzana Likar. There are some key differences between Fraxel CO2 (or any CO2 laser) and Fraxel Dual. For starters, CO2 lasers are very aggressive and can broadly treat a wide range of skin problems. It can take up to three weeks to recover from a CO2 laser treatment, and many patients don’t feel comfortable in social settings during this time. On the other hand, Fraxel Dual is programmed to treat specific issues and targets on the skin. It’s a non-ablative laser that heats up the tissue below the skin’s surface in order to stimulate collagen production. The top layer of the skin is left intact. Your skin may be pink for up to two days after the treatment on average, but it looks like you spent a bit too much time in the sun rather than having “had something done.”

What is non-ablative? Let’s look at another Fraxel laser for comparison. Fraxel Repair is ablative, and works by removing the top layer of skin while also stimulating collagen synthesis below the surface. As you can imagine, this is going to call for a significant recovery period. Fraxel Dual is simply gentler, but you’re going to need multiple sessions to get and keep the results you want. That works well for many people, especially those who prefer lunchtime procedures that fit into their busy schedule. Also bear in mind that with an ablative laser, there is a risk of infection and/or scarring.

Fraxel is one of the best-known lasers on the market, and at Palo Alto Laser and Skin Care we exclusively offer Fraxel Dual. What makes this particular laser so great? It’s a non-ablative option for overall skin resurfacing. Fractional lasers treat every skin zone while leaving the nearby areas untouched. It truly treats just a “fraction” of your skin at a time, which means a much quicker recovery compared to the lasers of yesteryear.

Fraxel Dual is designed to treat damaged and aged skin by creating many microscopic treatment zones deep below the skin’s surface. When old, hyperpigmented cells are vanquished, Fraxel sends heat into the skin to tighten it while also kick-starting collagen production. “Non-ablative” refers to the comparably low energy Fraxel Dual produces (compared to “ablative” lasers). Ablative lasers, which were common in the past, damage and remove the top layer of skin—usually almost entirely. As you can imagine, this leads to a painful and long recovery period. A good example is in the 1990s episode of “Sex and the City” when the character Samantha Jones gets a treatment that made her hide under a “bee keeper hat” for weeks. Thankfully, such recoveries are no longer necessary with options like the Fraxel Dual.

Fraxel History

Some clients ask our aestheticians what the difference is between Fraxel Dual and Fraxel re:store. Fraxel Re:pair was (is) ablative—Fraxel Dual is not. Fraxel Dual is actually the next generation of the Fraxel Re:store, and it still has the option of two different treatment wavelengths or “modes.” There is the 1550nm to penetrate into the deep layers of the skin to address deep lines, creases, and scarring (this mode is particularly helpful for many acne scars). The 1927nm isn’t as deep of a penetration, so it can be used for more surface-level corrective concerns such as poor tone, texture, and many types of skin damage. Many clients benefit from using both modes in different areas to treat a myriad of concerns.

From stretch marks to surgical scars and uneven skin tone to early signs of aging, Fraxel Dual is a comprehensive tool that can (almost) do it all. However, the Fraxel Dual isn’t a one-and-done treatment. Most clients need up to six initial sessions spaced a few weeks apart to get optimal results. After establishing a Fraxel Dual foundation, maintenance sessions may be recommended every few months so you can keep your results. Preparing for Your Fraxel Dual Treatment

Little prep is needed for Fraxel Dual, though just like with any laser treatment it is important to avoid direct UV exposure for two weeks before and after your treatment. Now is a great time to upgrade your sunscreen regiment to a medical-grade product. At your appointment, the treatment area will be cleansed and the laser will be passed over the area. The laser beam will overlap, horizontally and vertically, to ensure full coverage. Most clients describe the sensation as prickling or mild burning, and most do not require any numbing cream. However, if you have sensitive skin or if Fraxel Dual is being performed in a sensitive area, topical numbing agents are certainly available.

The length of your appointment depends on the area(s) being treated, but most take no longer than 30 minutes. A coolant is applied after the treatment to soothe the skin and help with recovery. Although skin can look red and irritated after the treatment, it is not typically severe enough to disrupt daily activities. If you have a special event coming up, it’s a good idea to plan your Fraxel Dual session at least five days in advance so any lingering redness can subside.

You might start to notice loose skin, or skin laxity, as early as 30 years old. It is one of the most common complaints we hear at Palo Alto Laser and Skin Care, and why we offer a variety of treatment options. Skin laxity is caused by a decrease in collagen production, decrease in “good fat” in the face (which is what causes that hollowed appearance), as well as gravity and genetics. You might also experience loose skin if you have lost a significant amount of weight or have experienced weight fluctuations throughout your life.

No matter the cause of your skin laxity, you have treatment options. However, keep in mind that at a certain point, laxity might be so severe that only surgical intervention is recommended. Until then, take a look at our top recommended non-invasive and minimally-invasive options:

1. Lasers!

Laser skin rejuvenation is one of the best ways to tighten the skin while also taking care of a host of other common problems like hyperpigmentation and poor tone. We have one of the largest collections of lasers in the region with options for all skin tones. Two great options for skin tightening include the Fraxel Dual and CO2 lasers. You can expect some pinkness after these treatments, but the downtime is quite brief and easy.

Alternatively, gentler lasers like broadband light (BBL) and Clear + Brilliant can also help with skin tightening. However, these are not as aggressive as some other lasers (in fact, Clear + Brilliant is often recommended as a pre-juvenation treatment for those in their twenties). The right laser for you will be decided in collaboration with your aesthetician. Keep in mind that laser treatments are not usually “one and done,” but rather require multiple treatments, with routine maintenance sessions, for the best results.

2. Dermal fillers.

Sometimes the best way to correct skin laxity isn’t by tightening the skin, but rather by filling in the lost volume. When we start to lose the “good fat” in our face and tops of our hands, it can lead to a witch-like, hollow appearance. When lost volume is the culprit, tightening the skin (either through surgery or lasers) will not give you the results you want. Instead, a dermal filler such as a hyaluronic injectable (HA) like Juvederm can restore a youthful appearance.

There are also “fillers” that don’t provide immediate results like HA based ones, but instead work via collagen induction therapy. They take some time for results to show, but offer results that last longer than HA fillers (up to two years).

3. Botox.

Botox to freeze lines and wrinkles that highlight skin laxity. In some cases, you can’t tell skin is lax if wrinkles, lines, and creases are gone! Botox isn’t technically a skin tightening treatment, but it can address the end result of lax skin. Many people consider Botox a staple maintenance treatment with injections needed every 3 – 6 months depending on the treatment site. You can also use Botox as a preventative measure, training muscles to avoid forming lines in the first place.

4. Thermage FLX

Thermage FLX is all about skin tightening. It works via collagen induction therapy, heating up the deeper layers of tissue without harming the superficial layer. Designed specifically to tighten skin on the face and body, most people need just one treatment to get the best results (which last up to two years). This treatment is exclusively used to tighten and tone the skin with results so significant that it has been dubbed a surgical lift alternative.

Clear + Brilliant

Clear + Brilliant laser is very popular for those in their twenties who want “pre-juvenation” and treatment of superficial concerns (such as mild skin dullness). Clear + Brilliant is suitable for all skin tones and types and has such a light touch that many clients opt for monthly treatments. There is no downtime and although your skin may look a bit sun-kissed after your treatment, most people immediately get back to their day. This laser is an excellent “entry” technology for those new to laser skin rejuvenation and will leave skin brighter and clearer with every treatment. However, it is not capable of tackling more advanced skin issues like signs of aging and moderate sun damage.

Broadband light (BBL)

BBL is the second generation of intense pulsed light (IPL) lasers and is popular for being able to treat over a dozen skin conditions ranging from lines and wrinkles to rosacea, melasma, sun damage, skin laxity, and even spider veins. BBL is also suitable for every skin tone and type, and is beloved by clients of all ages. If you are using BBL to get rid of unwanted hyperpigmentation like sun damage, you can expect the spots to get darker in the days following the treatment before they slough away like coffee grounds. Skin can look pink or red for up to 48 hours after your treatment, but this is often not intense enough to warrant “downtime.” However, you will need between 4 – 6 treatments on average, each spaced 2 – 4 weeks apart, to achieve optimal results in most cases. After this series, maintenance sessions every six months are recommended.

Morpheus8 Fractional RF Microneedling

Morpheus8 isn’t exactly a laser skin treatment, but rather a form of microneedling that takes results above and beyond anything you’ve experienced before. Since it’s much more than microneedling—and uses RF—Morpheus8 is often clumped in with the laser category. This is one of the latest technologies and has quickly become a client favorite. Unlike traditional microneedling, which causes micro-channels in the skin by expertly-applied needles, Morpheus8 uses specialized gold-plated needles that deliver RF directly into the skin. RF is a fantastic form of collagen induction therapy and is one of the best energies available to treat a wide variety of skin conditions. This hybrid approach is really two treatments in one. Some redness is common for a day or two post-treatment but, again, this typically doesn’t interfere with most daily tasks.

The area around the eyes is where most people first see signs of aging and sun damage, with many people noticing fine lines and wrinkles as early as their late teens. Those in their late teens and 20s are often drawn to Botox to both treat wrinkles and prevent them. This neuromodulator and its counterparts, such as Dysport, are a fantastic solution. Botox can be injected just about anywhere, including the corners of the eyes. However, bear in mind that the skill of your injector is equally as important as the treatment. Properly administered Botox injections look natural and allow for organic movement around the eyes. “Baby Botox,” or smaller units in delicate areas, is a great approach for younger clients with minimal signs of aging around the eyes.

Options for More Severe Wrinkles and Sagging

Those in their 30s and better often need something stronger than “just” Botox, such as lasers and Morpheus8. Not every laser and laser-aligned treatment is suitable for treating the delicate eye area. The term laser can mean a lot of things—and sometimes it doesn’t refer to a laser at all. It is more of an umbrella term to encompass a variety of technologies to treat a myriad of skin conditions from lines and wrinkles to laxity and hyperpigmentation. Clear + Brilliant

Clear + Brilliant is one of the mildest, gentlest lasers available and is often recommended for those in their 20s as a “pre-juvenation” treatment, but can also be an effective tool for skin that is a little more seasoned. This diode laser treatment might be for you if you’re starting to notice light lines around the eyes and mouth, typically the first place you’ll see them. Have lines on the cheeks? Notice that your skin texture feels rough? Been sporting some light “age spots?” These are all problems that Clear + Brilliant can treat.

Even though this is a gentle treatment, skin will be pink right after the treatment. You can expect this with any laser treatment. It usually goes away in a few hours. In the days following your treatment, skin might feel rougher and gritty while the superficial layer heals. However, just one week later, you’ll see that skin is brighter, more youthful, and softer. Most people opt for ongoing Clear + Brilliant treatments to keep those results as well as delay other signs of aging.

BBL

BBL, otherwise known as broadband light therapy, is a type of energy-based treatment that can brighten and enhance the skin while also stimulating collagen production. Collagen is what makes our skin youthful and plump in the first place, but we lose the ability to produce as much of it as we age. BBL is most often used on the face, neck, chest, arms, and hands, though it can actually be used anywhere. If you’re using BBL for hyperpigmentation, you’ll see dark spots (moles, etc.) get darker in the days following the treatment. Don’t be concerned. This is part of the process and the lesion will soon slough away (looking similar to coffee grounds). If you see that hyperpigmentation after your treatment, that’s a sign that it’s working. Recovery is similar to Clear + Brilliant, with some redness but it does not last long.

Fraxel

Fraxel lasers might be the name you most recognize. It treats a wide variety of skin woes, working to peel the superficial layer of skin away to reveal younger-looking, clearer, glowing skin. From severe acne scarring to more substantial lines and wrinkles, Fraxel is the most powerful of this trio. The treatment sites can be just about anywhere, but like with BBL the most commonly treated areas are the most visible. This laser comes with two wavelengths in one. The first targets texture, scarring, and wrinkles while the other takes care of hyperpigmentation and sun damage. The Fraxel family has a few different options, each designed for different issues.

Those with more melanin have long had to be careful when it comes to laser skin rejuvenation, lest unwanted skin lightening should occur. However, it is not just getting matched with the right laser (or lasers) that is critical for a positive experience. The skill of the providers matters a lot, too. Anyone can struggle with hyperpigmentation, dark spots that might be caused by any number of factors including sun damage. Often, Fraxel dual is recommended for treating hyperpigmentation, but this popular choice can be high-risk for those with darker skin tones. When those with more melanin are treated with Fraxel dual or ablative lasers, hypo- (lightening) and/or hyper- (darkening) pigmentation can occur. In this case, this includes those who are a Fitzpatrick skin type 4 or 5. There have been instances when unskilled and under-educated providers use the “wrong” laser on this skin type, resulting in more of the exact complexion you are trying to lessen.

Defining Ablative Lasers

Ablative lasers are a more aggressive laser option. They are designed to damage and ultimately destroy certain skin cells on the top two layers of skin—which means a recovery period is required. This leads to stimulated cell generation. However, the Fraxel laser is a non-invasive treatment featuring microscopic lasers that rejuvenates the complexion. It’s a fractionated laser, and these types of lasers can be ablative or non-ablative. Fraxel dual is non-ablative—so why can’t it be used on darker skin?

The Fraxel dual laser has two wavelengths (thus its name). However, since it generates heat, that heat can in turn kick-start melanocytes (the skin cells that produce melanin), resulting in even more melanin, otherwise known as post-inflammatory hyperpigmentation. In severe cases, the opposite can occur. Melanocytes can also be destroyed, making pigment levels “drop” and hypopigmentation occurs. Studies have shown that using some lasers on darker skin tones can also lead to swelling, prolonged redness, and acne breakouts.

Lasers for Those with Melanin

In years past, a well-informed provider would not use any laser on darker skin tones because the technology simply hadn’t caught up yet. That is no longer the case. One of the best, newer lasers for all skin types and tones is Clear + Brilliant. It can be used as a pre-juvenation treatment to keep skin looking healthy and youthful, or it can be used as a treatment for a variety of common skin conditions including brown spots and fine lines.
