Microneedling is both one of the oldest beauty techniques and one of the most popular procedures at Palo Alto Laser and Skin Care. It has its roots in ancient China with acupuncture, specifically in the book The Yellow Emperor’s Classic of Internal Medicine (100 B.C.E.). However, the goal of “microneedling” at that time wasn’t necessarily collagen induction therapy, at least not in those terms. Acupuncture was a means of opening conduits so that the Qi (life force) could flow more easily. Today, acupuncture is still used as a tool for promoting self-healing—which is what microneedling does! It’s possible that the collagen induction stimulated by ancient acupuncture revealed itself in improved skin which, of course, is a sign of good health.

That’s the origin story of microneedling, but microneedling as we know it today was first used in 1905 by a German dermatologist. Ernst Kromayer used a variety of dental burs (a tiny drill) with a flexible cord to treat hyperpigmentation, scars, and birthmarks. It quickly came to be known as a useful tool in helping war veterans with scarring. We have, of course, come a long way since the days of Kromayer literally drilling patients, but the concept is the same. When you create a controlled trauma, the result is a surge of collagen synthesis at that site.

Modern Day Microneedling

Microneedling as we would recognize it today started in 1995 with Dr. Desmond Fernandes. This Philadelphia-based dermatologist used hypodermic needle injuries to help with scars and wrinkles. Morepheus8 is the next generation of microneedling, combining the collagen induction therapy of traditional microneedling with the power of radiofrequency (RF) energy. A recent study, “Dermatologic facial applications of Morpheus8 fractional radiofrequency microneedling” took a look at the effectiveness and safety of one of the latest non-surgical options for facial lifts and tightening. From scarring to laxity and acne, collagen induction is known for helping with a variety of issues. Here’s what the researchers found.

RF technology works by emitting a low-frequency electromagnetic wave, creating an electromagnetic field in the skin. Then, this field creates dermal thermal heating, which in turn promotes elastin and collagen synthesis as part of the natural healing process. Different parts of the body (and different people) respond slightly differently, which is why Morpheus8 can be customized for optimal temperatures during each appointment. There needs to be specific degrees of heat in order to kick-start collagen production. Chromophore targeting is a predecessor of sorts to RF technology, but RF can target energy with a much lower risk of causing hyperpigmentation and injury. Another, much larger, study of 247 people revealed a “statistically significant improvement” for every single participant. They used the Baker Face/Neck classification rating for this study. Additionally, 93 percent of them said they were satisfied with their post-treatment improvement. The researchers found that those best suited for Morpheus8 were not interested in dermal fillers or Botox for the most part, and many of them already had tried fillers but remained unhappy with their skin laxity. Researchers noted, “In the age of excessive and improperly placed fillers used to achieve facial lifting and tightening, the demand for RF technology has exponentially increased in those for whom the additional volume is not recommended.”

Unfortunately, there have been a number of recent cases where people attempted DIY microneedling—either through buying an at-home “kit” or completely taking a DIY approach (including making the microneedling tool). Although microneedling is considered a minimally invasive procedure with no downtime, using the wrong equipment or performing microneedling without the required training and experience can be futile at best and cause permanent damage at worse.

Microneedling was first used at the turn of the last century, designed to undo severe scarring and skin damage (typically caused by warfare). It is one of the earliest forms of collagen induction therapy and works by creating numerous micro-channels (also known as micro-injuries) in the skin. When the skin is damaged, it “tells” the body to start producing collagen at the injury site in order to heal. This is true of all kinds of injuries, including controlled injuries that are invisible to the naked eye. Sometimes small areas of pinprick bleeding occur with microneedling, but this subsides by the time the client leaves. Your skin may be pink for a few hours, but otherwise there is no sign of injury—however, below the skin’s surface, your body is hard at work increasing collagen production and, in turn, healthier more youthful skin.
