During the summer months, many people want to avoid treatments that can make skin hyper-sensitive to the sun such as chemical peels. That’s a smart move, and at Palo Alto Laser and Skin Care we offer a variety of treatments that are summer-friendly. The triple threat of injectables can be used year-round, and if you’ve only tried one of them before it might be time to see what all three can do. First up is Botox, the gold standard in stopping wrinkles in their tracks. However, there’s a lot more to this neuromodulation.

Botox has been around for decades and works by temporarily freezing or paralyzing targeted muscles that cause lines, wrinkles, and folds. Increasingly, it has been being used by younger clients for preventative measures. If you can stop lines and wrinkles from prematurely forming—prevention bests a cure every time. Botox can also be used to produce the illusion of a fuller upper lip while masking a gummy smile. This is done via a lip flip, where small amounts of Botox are injected into the upper lip area. Results only last about three months on average since this is a very mobile area. Dermal Filler Fun

Dermal fillers are usually made of synthetic hyaluronic acid (HA), with natural HA already occurring in our bodies. HA draws moisture to the area, making it a very natural-looking filler. The results last anywhere from 6 – 18 months on average depending on the type of filler, the injection site, and the number of syringes used. Injectables refers to minimally invasive treatments that “inject” clients with a variety of products. One of the best-known injectables—and most requested at Palo Alto Laser and Skin Care—is Botox. However, just because Botox has been around for decades doesn’t mean you should trust just anyone with this treatment. The skill of your injector matters just as much as getting matched with the right treatment. Botox is best known for stopping lines, wrinkles, and creases in their tracks. This is achieved by temporarily “freezing” the muscles that cause these unwanted signs of aging. Botox is the most-used neuromodulator and results typically last for 3 – 6 months depending on the injection site and number of units used. Younger clients are increasingly using Botox as a preventative measure to train muscles not to create such deep lines in the first place. However, there are also a myriad of off-label uses for this injectable.

Turning Back the Clock

Botox can do a lot more than address wrinkles. It is also an excellent tool for giving a slight arch to the eyebrows. You can also use small amounts of Botox for a “lip flip,” which essentially flips the upper lip outward so that more vermillion shows. This can help hide a gummy smile and provides the appearance of fuller lips without actually increasing volume. However, for those who do want more voluminous lips—or more volume anywhere on the face—dermal fillers are most often the best option.

The majority of dermal fillers are made with a synthetic form of hyaluronic acid (HA). The HA our body naturally produces attracts water, which is why HA is associated with youthfulness and plumping. The results from dermal fillers are immediate, though some slight swelling and redness is common. Fillers, like Botox, are also temporary, but you still want to make sure you get results you love. “Duck lips” and “trout pout” might be short-term, but since most fillers are in highly visible places you want natural-looking results. An injector without the training, skills, and experience might give you an outcome that is poor and very fake looking.

The Latest in Injectables

There are a few dermal “fillers” that aren’t fillers at all. Instead, they work via collagen induction. Results are not immediate, and it can take up to six months to see results, although slow and steady improvement in volume will be noticed during this waiting period. The benefits of such fillers are that they tend to last longer (up to two years on average compared to the 6 – 18 months offered by HA fillers). Plus, a handful of clients prefer an “all natural” approach to volume improvement. You can also achieve collagen induction with a myriad of other treatments, such as microneedling. Non-HA fillers are less common and newer than HA fillers.

Another recent development in the world of injectables is Kybella. This procedure relies on a series of synthetic deoxycholic acid injected using a specialized stamping device. It treats unwanted fat in the submental (under-chin) area. On average, about 3 – 6 cycles spaced one month apart are needed for most clients. Conservative injections are given because it’s crucial not to over-correct. Once fat is gone with Kybella, it’s gone for good. Nearby fat cells can expand if weight gain occurs, but the specific cells burned with Kybella will not return. It’s important to know if double chins are caused by fat, sagging skin, or both. This can be discussed during your consultation, which is always complimentary. If sagging skin is also a problem, this can be addressed with a variety of in-office skin tightening treatments.

A 2021 article in Journal of Oral Biology and Craniofacial Research evaluated the longevity and effectiveness of using Botox “in the management of a gummy smile”—in other words, a lip flip. This is a popular procedure at Palo Alto Laser and Skin Care, though, of course, using Botox to prevent and manage lines and wrinkles is much more common. The researchers considered a longitudinal study with a four-year follow-up to gauge whether or not the lip flip was ultimately “worth it,” though a four-year follow-up might be considered foolishly hopeful. Botox for wrinkles lasts six months on average, and a lip flip typically lasts 3 – 4 months. Botox is designed to be temporary and doesn’t last as long in mobile places like the lips.

We know the lip flip works and we know the average timeframe of these results. What is of interest is what the research showed when it comes to assessing if this minimally invasive procedure might be useful as the only treatment to reduce a gummy smile. Subjects included in the study were all willing to get the lip flip, regardless of whether a surgeon had suggested another method. In total, there were 32 subjects divided into two categories. Group one had a gummy smile less than 5mm and were treated with three units of Botox. The other group had a gummy smile larger than 5mm and they received five units of Botox. Each group had two cycles within seven months (a proper timeframe) and kept in touch with researchers for the first 14 months.

Flipping Out Over Botox

All patients tolerated the procedure well according to researchers. There were no allergic reaction or antibody formation associated with the treatment. Both groups demonstrated “excellent” results for three months after the injection. At that point, all reported their gummy smile slowly returning. The authors note that “significant changes” appeared at the five-month mark. This is in keeping with what is already known about the lip flip and while it is not surprising to providers that the lip flip gives fantastic results and requires quarterly treatments, it might come as a pleasant surprise for those looking for an alternative or complement to lip fillers.

In conclusion, the researchers “recommend use of BT [Botox] for the treatment of a gummy smile as the technique is safe, economical and easy to use.” They point out that it is not long-lasting and that it might encourage patients to opt for a surgical fix in the future. However, there are options other than surgery for this common condition. Dermal fillers and “fillers” that often fall into this category but are actually collagen induction therapy can also work. It all depends on the severity of the gumminess, the client, and the look they want to achieve.

Botox Does it All

The lip flip is just one of the many uses for Botox. Increasingly, younger and younger patients are opting for the neuromodulator in order to prevent wrinkles before they form. You can train the muscles causing furrows to be weaker and, in turn, less able to create lines. Botox is also commonly injected along the browline to create a slight arch, which can be a fantastic alternative to a minor brow lift. This area is not nearly as mobile as the lips, so results tend to last longer.

There are also many non-aesthetic uses for Botox, such as using the neuromodulator to stop excessive sweating, prevent and treat migraines, and recent studies have even shown that it can help reduce severity of anxiety in those struggling with this common mental health concern. Botox can seemingly do it all, but remember that the skill of the injector is also of utmost importance. You might start to notice loose skin, or skin laxity, as early as 30 years old. It is one of the most common complaints we hear at Palo Alto Laser and Skin Care, and why we offer a variety of treatment options. Skin laxity is caused by a decrease in collagen production, decrease in “good fat” in the face (which is what causes that hollowed appearance), as well as gravity and genetics. You might also experience loose skin if you have lost a significant amount of weight or have experienced weight fluctuations throughout your life.

No matter the cause of your skin laxity, you have treatment options. However, keep in mind that at a certain point, laxity might be so severe that only surgical intervention is recommended. Until then, take a look at our top recommended non-invasive and minimally-invasive options:

1. Lasers!

Laser skin rejuvenation is one of the best ways to tighten the skin while also taking care of a host of other common problems like hyperpigmentation and poor tone. We have one of the largest collections of lasers in the region with options for all skin tones. Two great options for skin tightening include the Fraxel Dual and CO2 lasers. You can expect some pinkness after these treatments, but the downtime is quite brief and easy.

2. Dermal fillers.

Sometimes the best way to correct skin laxity isn’t by tightening the skin, but rather by filling in the lost volume. When we start to lose the “good fat” in our face and tops of our hands, it can lead to a witch-like, hollow appearance. When lost volume is the culprit, tightening the skin (either through surgery or lasers) will not give you the results you want. Instead, a dermal filler such as a hyaluronic injectable (HA) like Juvederm can restore a youthful appearance.

There are also “fillers” that don’t provide immediate results like HA based ones, but instead work via collagen induction therapy. They take some time for results to show, but offer results that last longer than HA fillers (up to two years).

3. Botox.

Botox to freeze lines and wrinkles that highlight skin laxity. In some cases, you can’t tell skin is lax if wrinkles, lines, and creases are gone! Botox isn’t technically a skin tightening treatment, but it can address the end result of lax skin. Many people consider Botox a staple maintenance treatment with injections needed every 3 – 6 months depending on the treatment site. You can also use Botox as a preventative measure, training muscles to avoid forming lines in the first place.

4. Thermage FLX

The right treatment—or treatments—for loose skin depends on severity, location, and cause. If you think wrinkles are the primary signs of aging, that is only half of the equation. Palo Alto Laser and Skin Care often recommends combining Botox with dermal fillers because lost volume is another key sign of aging. It might not be as obvious as lines and wrinkles at first blush, but we all lose good volume and fat in our face as we get older. Voluminous, plump skin is an inherent sign of youth—which is why those with “baby faces” tend to look younger for much longer.

Botox, and its counterparts such as Dysport, have long been a client favorite because results are fast and dramatic, yet still natural-looking. Botox is a minimally invasive procedure that temporarily “freezes” the muscles that cause lines and wrinkles. It can also be used as a preventative measure to stop lines and wrinkles from forming. Lines are caused by repetitive muscle movement, which means they can be trained just like you train muscles in the gym. Botox and Fillers

Botox and fillers have been close bedmates for years because they tackle two very different signs of aging: wrinkles and lost volume. Botox and its counterparts can stop lines and wrinkles for up to six months or prevent them from happening (also known as pre-juvenation). A systemic review of clinical studies analyzes data derived from a lot of other related studies. One such study, conducted in 2019, took a lens to lip fillers—specifically how safe and effective they are. Dermal fillers, and lip fillers, in particular, have long been one of the most popular treatments at Palo Alto Laser and Skin Care. Hyaluronic acid (HA) fillers are the most common, providing immediate results that can last for several months depending on the anatomy treated, number of syringes, and type of lip filler. If you are considering lip fillers but worry about whether they really work and how safe they are, this study review will put your mind at ease.

Lip fillers are here to stay because lip volume is a key aesthetic factor when determining attractiveness for both men and women. Since fillers are here to stay, it’s important to understand how they work and their efficacy. The study review looked at 22 studies that included a total of 3,965 participants. According to researchers, these fillers “turned out to be an effective and safe treatment” overall. The most common “adverse” event reported was local reactions at the injection site, such as redness, swelling, and bruising, which can be expected with any injection (even a vaccine shot). However, these results are temporary and usually dissipate within a few days. The researchers report that the “majority of included subjects were satisfied with the result and their looks.”

Why Lip Fillers?

The study calls the boom of lip fillers a “social phenomenon.” Today, the vast majority of aesthetic procedures in the U.S. are non-surgical or minimally invasive (lip fillers fall in the latter category since they require an injection). Today, soft-tissue augmentation— the technical term for lip fillers—is often the most requested procedure. HA is a great choice because it has a high viscosity that mimics plumpness in other parts of the body (in fact, we make HA ourselves, but HA in fillers is a synthetic version). Most of our natural HA is in our skin, and the highest concentration can be found in connective tissue.

Some people blame Kylie Jenner and other such influencers for creating the demand for lip fillers, but people (women in particular) have been exaggerating their pout for centuries. Simply put, lips that are full and defined (both of which can be achieved with lip fillers) are linked to not only attractiveness, but youthfulness. Our lips change with time and age, getting thinner, deflated, and with borders that become ill-defined. However, the researchers remind us that aesthetic procedures require an expert understanding of anatomy, and especially the surface anatomy, which is why you shouldn’t trust just anyone with your lip fillers. Understanding the Face of Fillers

The face is naturally divided into three sections, and the lips are the key feature of the lower trio. It’s actually the upper lip that has a big impact on how we aesthetically judge faces. The “ideal” ratio is to have the lower lip just slightly larger than the top, but it has been well-documented that lips thin with age—and sometimes the upper lip can appear to disappear altogether. When this occurs, both lip fillers and a lip flip (using Botox) may be helpful.

In this review of studies, a total of 48 articles were examined. Abstracts were manually reviewed by a secondary author so that any articles that addressed non-related issues (such as cheek fillers) were discarded. Any studies that did not use HA fillers were also excluded. Not only did most patients report that they were satisfied with their results (96.9 percent), but 61 percent reported that they were still happy with results nine months after their treatment. In total, almost 80 percent were satisfied six months after their treatment. The dwindling in satisfaction is likely due to the HA “wearing off” and perhaps the patients not being matched with the right lip filler.
